Microchipping your pet greatly improves the chance that you'll get your pet back if it becomes lost or stolen.
Did you know?...In Maumelle, the annual pet license fee for pets that are spayed or neutered and have a registered microchip is only $5!
PLEASE Read the "Fine Print":

ALL dogs must be on a leash! NO harnesses please. Chips cannot be inserted if the pet is wearing a harness and removing a harness greatly increases the chance of the pet getting loose.

ALL cats must be in a pet carrier.

Proof of current rabies vaccine required (bring papers or tag, even if The Boulevard Vet is your vet...they will not have staff available who can look up your records).

Please muzzle your dog if it is not friendly or is afraid of vets.

MFOA will be registering your pet with PetLink for you.
Microchipping Event Registration Form*
This information is required to register your pet's microchip per PetLink. All information gathered is to assist in successfully reuniting you and your pet should they become lost and subsequently found by someone else.
Maumelle Friends of the Animals does not sell information collected and will only share your personal information with PetLink as necessary to operate their services.
*Only 55 microchips are available. Once we have 55 spots filled, registration will be deactivated.
*Only one pet per registration.
*ONLY ONE REGISTRATION PER ANIMAL PLEASE! If you need to correct information, contact us via email and we can make corrections, do not resubmit a second reservation for the same animal.
